Observed problem

The kit now tells agents to create focused tracker issues for actionable follow-up work, but it does not yet provide a reusable issue template that standardizes those reports.

Impact

Humans and multiple agents can collaborate more reliably when follow-up issues have consistent fields for context, affected files, next steps, and verification.

Verification performed

Created as a live test of the follow-up issue guidance after the local GITEA_TOKEN was refreshed. No sensitive data or logs are included.
